Customized library instruction sessions may be requested by completing the following form indicating the librarian of choice and/or the subject area, database, or research tools desired. Please allow at least two weeks for arrangements to be made. The instructor/requestor will be contacted to confirm the date, time, location, and content of the instruction session.

PLEASE NOTE: A confirmation contact from the librarian for the date, time, and location will be your notification that the class session you requested is actually scheduled.
